Bruce Bagley, MD, FAAFP, serves as president and CEO of TransforMED, a wholly owned subsidiary of the American Academy of Family Physicians. As president and CEO, Dr. Bagley serves as the leader, visionary, and key spokesperson for the company. Frederick Bloom, Jr., MD, MMM

Sean Cavanaugh

Sean Cavanaugh is the Deputy Administrator and Director of the Center for Medicare at the Centers for Medicare & Medicaid Services. He is responsible for overseeing the regulation and payment of Medicare fee-for service providers, privately-administered Medicare health plans, and the Medicare prescription drug program. Medicare provides health coverage to 50 million elderly and disabled Americans, with an annual budget of over $550 billion. Prior to assuming his current role, Sean was the Deputy Director for Programs and Policy in the Center for Medicare and Medicaid Innovation. In that capacity, he was responsible for overseeing the development and testing of new payment and service delivery models, including accountable care organizations and medical homes. Previously, Mr. Cavanaugh was director of health care finance at the United Hospital Fund in New York City. He has also served in senior positions at Lutheran Healthcare (Brooklyn, NY), the New York City Mayor's Office of Health Insurance Access, and the Maryland Health Services Cost Review Commission. He started his career on Capitol Hill working for a member of the Ways and Means Health Subcommittee. He attended the University of Pennsylvania and the Johns Hopkins School of Hygiene and Public Health.

Kenneth R. Cohen, MD, FACP

Dr. Kenneth Cohen has served as the Chief Medical Officer of New West Physicians since 1996. He is an experienced physician leader, practicing physician, and researcher who has attained local and national recognition for clinical research, internal medicine, and health care quality improvement. He has successfully developed, directed, evaluated and reported numerous clinical quality studies in primary care, including tobacco cessation, osteoporosis, asthma, diabetes, hypertension, and ischemic vascular disease. He has presented at national and local conferences and symposia, and has authored numerous publications and reports. He is a Clinical Associate Professor of Medicine and Pharmacy at the University of Colorado School of Medicine. He was recognized as the Exempla Lutheran Medical Center Physician of the Year in 2010. Paul Ginsburg is Norman Topping Chair in Medicine and Public Policy at the University of Southern California, where he is affiliated with the Schaeffer Center for Health Policy and Economics. Until the end of 2013, he was President of the Center for Studying Health System Change (HSC), which he founded in 1995. A doctor and economist by training, McClellan has a highly distinguished record in public service and academic research. McClellan served as a member of the President's Council of Economic Advisers and senior director for health care policy at the White House under President George W. Bush. Dr. Nguyen is a board-certified family practice physician. Alice M. Rivlin is the Director of the Engelberg Center for Health Care Reform, the Leonard D. Schaeffer Chair in Health Policy Studies and a Senior Fellow in Economic Studies at the Brookings Institution. She is also a Visiting Professor at the Public Policy Institute of Georgetown University. In 2010 President Obama appointed Rivlin to the Simpson-Bowles Commission on the federal budget. Ms. Struk's diverse nursing practice includes a variety of clinical and operational positions, both in hospital and community settings. She began her home health career as a pediatric nurse practitioner implementing home care programs for patients with complex conditions. She has held positions in clinical management, quality, hospice operations, and chief operating officer in both independent and hospital based non-profit agencies. She holds a PhD in Health Services Research from Case Western Reserve University in Cleveland focused on quality and predictive modeling. She maintains faculty positions at Cleveland State University and CWRU. Her interest is concentrated on improving clinical practice through quality improvement efforts, better care delivery models and the use of evidence based practice across care delivery systems.

Gail R. Wilensky, Ph.D. is an economist and senior fellow at Project HOPE, an international health foundation. She directed the Medicare and Medicaid programs and served in the White House as a senior adviser on health and welfare issues to President GHW Bush. She was also the first chair of the Medicare Payment Advisory Commission. Her expertise is on strategies to reform health care, with particular emphasis on Medicare, comparative effectiveness research and military health care. Dr. Wilensky currently serves as a trustee of the Combined Benefits Fund of the United Mine Workers of America and the National Opinion Research Center, is on the Board of Regents of the Uniformed Services University of the Health Sciences (USUHS), the Visiting Committee of the Harvard Medical School and the Board of Directors of the Geisinger Health System Foundation. She is an elected member of the Institute of Medicine, served two terms on its governing council and currently chairs the Healthcare Servicing Board. She is a former chair of the board of directors of Academy Health, a former trustee of the American Heart Association and a current or former director of numerous other non-profit organizations. Dr. Wilensky testifies frequently before Congressional committees, serves as an adviser to members of Congress and other elected officials, and speaks nationally and internationally. She received a bachelor's degree in psychology and a Ph.D. in economics at the University of Michigan and has received several honorary degrees.

Her background includes operations management, project and program management, patient experience, Lean Production Systems/process improvement, quality improvement, and value-based risk contracting. She received her master's degree in Health Services Administration from the University of Michigan, School of Public Health in 2006.
