Bio: Keith Alexander has worked for more than 30 years in the healthcare industry, joining Memorial Hermann as CEO in 2007, following three years as chief operating officer at Saint Joseph HealthCare in Lexington, Ky. His experience also includes operational and administrative executive roles at Intermountain Health and HealthTrust in Utah. He began his career as an emergency room technician. He holds a BS in Biology and an MHA from Ohio State University.
Keith Fernandez, MD, MHMD

Bio: In his role as president and physician-in-chief of MHMD, the Memorial Hermann Physician Network, and chief medical officer of the Memorial Hermann ACO, Dr. Fernandez leads the largest clinically integrated physician organization in Texas. A past chairman of the MHMD board of directors, he continues to serve in several physician leadership roles, including as chairman of the clinical programs committee, and is the Memorial Hermann physician champion for electronic order entry. A practicing gastroenterologist, Dr. Fernandez attended Florida State University and the University of South Florida, and earned his MD from Uniformed Services University of the Health Sciences Ebert School of Medicine.

Bio: Mary Folladori has more than 30 years of experience in the nursing profession as a clinician, care manager, and leader. Her accomplishments include leading the development of an innovative care coordination department for population health management within one of the country's largest ACOs. She has a background in the development of patient education materials, disease management and care coordination services for patients with complex medical conditions, and has served in leadership roles within physician-led IPAs in Colorado and Texas. Folladori earned her MSN and BSN degrees from the University of Northern Colorado.

Bio: Carl Josehart is an experienced healthcare executive with over 25 years as a clinical social worker, healthcare consultant, and executive in acute care and rehabilitation hospitals. He joined TIRR Memorial Hermann Hospital as CEO in 2006 and has served since 2011 as system executive for rehabilitation services. Josehart is also clinical assistant professor in the Departments of Physical Medicine & Rehabilitation at the University of Texas and Baylor College of Medicine. He holds a bachelor's degree in psychology from Washington University and a master's degree from the George Warren Brown School of Social Work.
Kathy Johnson, RN, BSN

Bio: Kathy Nipper Johnson has been director of acute-care case management for over a decade, having previously served as the administrator for case management and social services programs for Florida Hospital Adventist system and Baptist Memphis. Earlier in her career, she worked as a nurse for a primary care pediatric office, the State of Florida with a pediatric AIDS program, and with the Multihandicap Assessment Team (MHAT) in Florida. She completed nursing training at Central Missouri State University.

Bio: An RN, Susan Markland has specialized in hospice and homecare programs since 2001 and has worked with other large systems, including Scott & White and CHRISTUS Health Care. She has led multiple home-based services, including home health, adult and pediatric hospice, telemonitoring, cardiac specialized teams, palliative care, care transitions, a Medicaid home waiver program, private care services, and the management of multiple rural health clinics. She holds a bachelor's degree from Texas A&M.

Bio: Pat Metzger provides support and case management direction to the system's 11 hospitals, and community case management operations, as well as home health, hospice, and sleep services. Under her leadership, the care management program was awarded the Franklin Award of Distinction in Case Management in 2011. Metzger also led the development and implementation of the chronic disease care management initiatives and Community Outreach for Personal Empowerment (COPE), including programs that demonstrated $20 million in cost avoidance for the system. Prior to joining Memorial Hermann, Metzger served as a principal in the healthcare consulting practice of Ernst & Young. A former chief nursing officer and chief operating officer, she has authored numerous articles and served as adjunct faculty in the masters' in nursing program at Ursuline College. She holds a bachelor's degree in nursing and a master's degree in healthcare administration.
Michael Shabot, MD

Bio: Dr. Shabot is responsible for system quality, patient safety, epidemiology, medical informatics, pharmacy, laboratory services, critical care, registry reporting/performance improvement, and nursing education. He also serves as chairman of the board for the Memorial Hermann ACO. A member of the board quality committee of the Mercy System in St. Louis, he also represents the American Medical Informatics Association on the CMS National Priorities Partnership. Dr. Shabot is an adjunct professor at the University of Texas at Houston in the Schools of Biomedical Informatics and Public Health. Under his leadership, Memorial Hermann received the National Quality Healthcare Award from the National Quality Forum, the National Patient Safety Leadership Award from the VHA Foundation and National Business Group on Health, and the John M. Eisenberg Award for Patient Safety and Quality at the National Level from The Joint Commission and National Quality Forum. The coauthor of Integrating Quality and Strategy in Health Care Organizations, Dr. Shabot earlier served as medical director of enterprise information services, director of surgical intensive care, and associate director of surgery at Cedars-Sinai Medical Center.
