Patricia L. Metzger, BSN, MSA, ACM, FABC
Bio: Chief, Care Management, Memorial Hermann Healthcare System Pat Metzger is a registered nurse with over 35 years of healthcare experience. She holds a Bachelor's Degree in Nursing and a Master's Degree in Healthcare Administration. She is a Fellow of The Advisory Board. She has served as a Chief Nursing Officer, as well as a Chief Operating Officer. She is currently the Senior Vice President, Care Management for Memorial Hermann Health System in Houston, Texas. Since joining Memorial Hermann she has been responsible for ongoing growth and development of the Care Management initiatives. Pat carried the Care Management program to national recognition with Memorial Hermann being awarded the Franklin Award of Distinction in Case Management in 2011. Prior to joining Memorial Hermann, Ms. Metzger served as a Principal in the Healthcare Consulting Practice of Ernst and Young. She has authored numerous articles and published The Case Managers Training Manual with Aspen Publications. She is a contributor to Peter Kongstevedt's work, The Essentials of Managed Care.
Christopher Lloyd, MD
Bio: Chief Executive Officer of MHMD Christopher Lloyd is the Chief Executive Officer of MHMD, the Physician Network for the Memorial Hermann Health System in Houston, Texas. MHMD includes the largest clinically integrated physician organization in Texas with more than 2,000 participating physicians. He is also Chief Executive Officer of the Memorial Hermann Accountable Care Organization (MHACO). The MHACO is one of the largest in the country and has generated the top Medicare Shared Savings Program (MSSP) results in the country for 2013 and 2014. It currently functions across the entire payor spectrum, including the MSSP, commercial and Medicare Advantage populations. Chris has over 25 years of executive experience in acute healthcare settings, including community acute care, academic hospital and physician group management. Prior to assuming his current role in 2009, Chris was the Chief Operations Officer at Memorial Hermann Hospital in the Texas Medical Center, as well as the Chief of Clinical Service Lines.
Paula Parsons Lenhart, RNC, MSN, CNS, ACM, FABC
Bio: Associate Vice President of Care Management at Memorial Hermann Health System Paula Parsons Lenhart, is Associate Vice President of Care Management for Memorial Hermann Health System which includes ten acute care hospitals, a children's hospital, two rehabilitation hospitals, 20 ambulatory surgical centers and 200 other ambulatory facilities. She is responsible for care management program development, education, oversight of clinical quality improvement and chronic disease programs. She serves as coach and mentor to case management directors and case managers in skill development and acts as a resource to hospital-based case management directors. She is also responsible for attaining optimal targeted clinical and financial outcomes through the care management process. Paula is a certified neonatal intensive care nurse, accredited case manager and fellow in The Advisory Board Company. She was recently recognized as Houston Chronicle's 2015 Top 150 Nurses and awarded the Memorial Herman Memorial City Medical Center Breakthrough of the Year Award for patient satisfaction improvement through interdepartmental collaboration for the diabetes center department. She is active in American Case Management Association and serves as President for ACMA Greater Houston Chapter. She is a guest lecturer at Houston Baptist University School of Nursing and instructor at Beyer State University master of case management program.
Mary Folladori, RN MSN
Bio: System Director of Care Management at the Memorial Hermann Physician Network (MHMD) Mary Folladori has more than 30 years of experience in the nursing profession as a leader, care manager and clinician. She leads an advanced and accountable Care Coordination department designed for Population Health Management within one of the country's largest and most successful ACOs. Mary's background in Case Management and Physician Organizations and Leadership roles within Physician led IPA's in Colorado and Texas fueled her passion for designing and creating community based teams of multi-disciplinary professionals committed to coordinating high quality care in a fiscally responsible manner. Ms. Folladori earned both her MSN, with an emphasis in Chronic Illness and Education and her BSN from the University of Northern Colorado.
Dr. Keith Fernandez
Bio: President and Physician-in-Chief, Memorial Hermann Physician Network Born in Spokane, Washington, Dr. Keith Fernandez attended Florida State University and the University of South Florida before receiving his medical doctorate from the Uniformed Services University of the Health Sciences Ebert School of Medicine in Bethesda, Maryland. In addition to caring for his patients, Dr. Fernandez serves as President and Physician-in-Chief of MHMD, the Memorial Hermann Physician Network and as Chief Medical Officer of the Memorial Hermann Accountable Care Organization (ACO). His responsibilities include a 2000 physician clinical integration program, 350 Advanced Primary Care Practices (APCP), and a care management group of 70 nurses, social workers and pharmacists that work as a part of the APCP. He is also chairman of MHMD's Clinical Programs Committee, which provides governance for quality, safety and cost effectiveness for the organization through 40 physician governed quality, safety, and supply chain committees.
Bio: Chief Community Benefits Officer, Memorial Hermann Healthcare System Carol Paret is the Chief Community Benefits Officer for Memorial Hermann Hospital System and CEO of Memorial Hermann Community Benefit Corporation. In addition to overseeing Memorial Hermann's community efforts, she is the privacy and security officer and has accountability for the medical records function, CME and a large family practice residency program. She did her undergraduate work at University of Houston and her graduate studies at University of Texas School of Public Health in Houston. Her career in healthcare began with a community health planning emphasis and for the past 31 years she has been with Memorial Hermann Healthcare System. She is currently serving as past Board Chairman of Gateway to Care, a collaborative involving 165+ agencies whose mission is to improve access to care for Houston's uninsured and the Provider Health Network, a project access model for specialty care.
Michael Shabot, M.D.
Bio: Chief Clinical Officer and Executive Vice President, Memorial Hermann Healthcare System, and Chairman, Memorial Hermann ACO M. Michael Shabot, M.D. is Executive Vice President and Chief Clinical Officer for the Memorial Hermann Health System in Houston, Texas, which includes ten acute care hospitals, a children's hospital, two rehabilitation hospitals, 20 ambulatory surgical centers and 200 other ambulatory facilities. He is responsible for Memorial Hermann's medical groups with 4,000 physicians, its Accountable Care Organization, a primary care residency, Health Information Management, Case Management, Care Management, Medical Informatics, Quality, Safety, Infection Control and Registry Reporting. In both 2013 and 2014 the Memorial Hermann ACO generated the highest savings the CMS Medicare Shared Savings Program has ever recorded, totaling over $111M. Dr. Shabot is an Adjunct Professor at the University of Texas at Houston in the Schools of Biomedical Informatics and Public Health. He serves on the Board Quality Committee of the Mercy System in St. Louis, MO. Under his leadership, in 2013 Memorial Hermann received the John M. Eisenberg Award for Patient Safety and Quality at the National Level, presented by the Joint Commission and National Quality Forum. Thomson Reuters rated Memorial Hermann one of the top 5 large US healthcare systems in 2012 and 2013. In 2012 the system received the Texas Hospital Association's Bill Aston Quality Award for its high reliability program. In 2009 Memorial Hermann received the National Quality Healthcare Award from the National Quality Forum as well as the National Patient Safety Leadership Award from the VHA Foundation and National Business Group on Health. The book Integrating Quality and Strategy in Health Care Organizations, coauthored by Dr. Shabot, was published in August 2012.
